Notice of Privacy Practices
Your Information. Your Rights. Our Responsibilities. This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
Your Rights
When it comes to your health information, you have certain rights. Subject to applicable law, you may:
- Ask to inspect or obtain an electronic or paper copy of your medical record and other health information we maintain about you.
- Ask us to correct health information you believe is incorrect or incomplete.
- Ask us to contact you in a specific way or at a specific location for confidential communications.
- Ask us to limit certain uses or disclosures of your health information. We are not required to agree to every request, but we will honor restrictions when the law requires us to do so.
- Ask for a list, or accounting, of certain disclosures of your health information.
- Ask for a paper copy of this notice at any time.
- Choose a personal representative to act for you when legally authorized.
- File a complaint if you believe your privacy rights have been violated.
Your Choices
For certain health information, you may tell us your choices about what we share. Depending on the circumstances and applicable law, this can include sharing information with family or friends involved in your care, disaster-relief situations, and certain communications about our services. Uses or disclosures that require your written authorization will not be made without that authorization. If you give us written authorization, you may revoke it in writing as permitted by law.
We will not sell your protected health information or use it for marketing in a manner that requires your authorization without first obtaining the authorization required by law.
How We May Use and Share Health Information
Treatment
We may use and share your health information to provide, coordinate, or manage your health care and to communicate with other health professionals involved in your care.
Payment
Where applicable, we may use and share health information to bill for services, obtain payment, or perform related payment activities.
Health Care Operations
We may use and share health information to operate the practice, improve care, train staff, conduct quality activities, manage business functions, and work with service providers that perform functions for us under applicable privacy requirements.
Other Uses and Disclosures Permitted or Required by Law
We may use or disclose health information when permitted or required for public health and safety activities, reporting suspected abuse or neglect, health oversight, legal and administrative proceedings, law enforcement purposes, workers’ compensation, organ and tissue donation, medical examiner or funeral director functions, research when legal requirements are met, national-security activities, or other purposes authorized or required by law.
Specially Protected Information
Some types of information may receive additional protection under federal or Texas law. When a law provides greater privacy protection than the general rules described here, we will follow the more protective requirement. This may include certain mental-health, genetic, communicable-disease, reproductive-health, or substance-use-disorder information when applicable to the information and services involved.
Our Responsibilities
- We are required by applicable law to maintain the privacy and security of protected health information.
- We will notify affected individuals as required if a breach occurs that may have compromised the privacy or security of unsecured protected health information.
- We must follow the duties and privacy practices described in the notice currently in effect.
- We will not use or share your information other than as described in this notice unless you authorize us in writing or another law permits or requires the use or disclosure.
Changes to This Notice
We may change the terms of this notice, and changes may apply to health information we already maintain as well as information we receive in the future. The current notice will be available upon request, at the practice, and on our website.
